Job summary

The Lakes Home Care, Inc. in Florida is seeking a licensed Nurse Practitioner (APRN) to support our home health operations. The NP will ensure compliance with Medicare guidelines, assist with Plan of Care documentation, and conduct Face-to-Face patient encounters.

The role offers a flexible schedule, an administrative-supportive team, and a growing agency with a strong compliance focus. Previous home health or geriatrics experience is preferred and PECOS enrollment is a plus.

Qualifications

• Active Florida APRN license is required.
• Active NPI number is required.
• Medicare enrollment (PECOS) preferred.
• Experience in Home Health or geriatrics preferred.
• Knowledge of Medicare guidelines and documentation is required.

Responsibilities

• Review and sign Plan of Care (POC) and recertifications.
• Perform Face-to-Face encounters as required.
• Support clinical documentation compliance (Medicare/AHCA).
• Collaborate with RNs, MDs, and administrative staff.
• Assist in reducing hospital readmissions through patient follow-up.

Compensation:
