Premise Health is hiring a Full Time Nurse Practitioner for our Health Center in Sunnyvale, CA. You will conduct physical exams, perform procedures, develop care plans, and educate patients while coordinating with physicians for comprehensive care.

Qualifications

• Licenses required to practice as Nurse Practitioner in target state.
• National certification in Nursing (ANCC or AANP) required.
• Master's Degree in Science of Nursing or equivalent.

Responsibilities

• Conducts physical exams and preventive health measures per guidelines.
• Performs procedures such as injections, suturing, wound care, and infection management.
• Develops patient care plans and ensures continuity of care.
• Educates patients on treatment plans and health maintenance.
• Orders and interprets diagnostic tests to assess clinical conditions.
• Collaborates with physicians and healthcare teams for comprehensive care.
• Prescribes or recommends treatments, including medications and therapies.
• Refer patients to specialists as needed.
• Maintain accurate and up-to-date patient records.
