Job Opportunity: Nurse Practitioner in Home and Community Care

$40,000 Student Loan Repayment or $20,000 Sign-on Bonus available for eligible individuals who have not previously participated in this program.

Optum is seeking a Nurse Practitioner to join our HouseCalls team in Allegheny & Washington, PA. We are a clinician-led care organization dedicated to creating a seamless health journey for patients across the care continuum.

As part of the broader Home and Community Care team, you'll conduct annual clinical assessments in patients' homes, helping to identify health risks and coordinate follow-up care to enhance health and well-being.

Our integrated medical teams are pioneering new approaches in healthcare, providing high-quality medical services and compassionate care to vulnerable populations. We address physical, mental, and social needs, ensuring patients access and navigate care effectively, anytime and anywhere.

This role offers a balanced work-life schedule with flexible hours over a 7-day week, no on-call, weekends, or holidays required.

Primary Responsibilities:
  1. Conduct in-home assessments including medical history, review of systems, medication reconciliation, vital signs, and physical exams.
  2. Perform screenings and point of care testing as appropriate.
  3. Identify diagnoses and contribute to care management plans.
  4. Document findings to inform the patient's primary care provider of any care gaps.
  5. Recognize urgent situations and intervene accordingly.
  6. Educate patients on disease processes, medications, and adherence.
Required Qualifications:
  • Active NP license or ability to obtain one by start date, free of disciplinary actions.
  • For NPs: Autonomous practice approval from licensing board if applicable. PAs: Ability to work under a Collaborative Agreement.
  • Certification from ANCC or AANP in Family, Geriatrics, Adult, or Adult-Gerontology Primary Care, or ability to obtain it.
  • Physical ability to perform assessments, including moving equipment and navigating various home environments.
  • Valid driver's license and reliable transportation for home visits.
Preferred Qualifications:
  • Over 1 year of clinical experience in relevant settings.
  • Additional licensure in other regions may be required.
  • Experience in gerontology, cardiology, internal medicine, or endocrinology.
  • Home health or home visit experience.
  • Ability to travel within the state, with potential for additional incentives.
  • Fluency in Spanish, Cantonese, Korean, Vietnamese, Polish, or other languages.

The salary range is $89,800 to $176,700 annually, based on full-time employment and various factors including experience and certifications. Benefits include comprehensive health coverage, incentive programs, stock options, and 401(k).
