Location: 3046 W. 127th St. Blue Island, IL 60406

Schedule: Full-Time, Monday to Friday, 8:00 AM - 5:00 PM

Language Requirement: Native/Bilingual Proficiency in Spanish

Role Overview

The Nurse Practitioner at Oak Street Health provides effective, equitable, value-based primary care to Medicare adults, aiming to keep them healthy and satisfied. They see patients independently, collaborate with physicians, and build long-term relationships to foster trust and understanding.

Work in outpatient settings with a collaborative team including physicians, nurse practitioners, PAs, medical assistants, and support staff. Partner with specialists and hospitals for comprehensive care.

Core Responsibilities

• Deliver exceptional primary care services.
• Conduct office visits for routine and acute issues.
• Perform annual wellness visits and health risk assessments with thorough documentation.
• Coordinate care with other providers and facilities.
• Lead population health initiatives in coordination with the care team.
• Assist with phone triage and outreach efforts.
• Educate patients on health conditions and care plans.
• Participate in promotional activities.
• Conduct home visits as needed.

Qualifications

• Master of Science in Nursing, DNP, or NP certification.
• National certification in Family, Adult-Gerontology, Adult, or Gerontological Nurse Practitioner specialties.
• Active, non-probationary state NP license.
• Active DEA license.
• US Work Authorization.

Preferred Skills

• Experience in primary care, geriatrics, or family medicine.
• Knowledge of HCC, ICD-10 coding, and HRAs.
• Team-oriented with a passion for population health.
