Role Overview

The Nurse Practitioner at Oak Street Health provides effective, equitable, value-based primary care to Medicare adults, aiming to keep them healthy, satisfied, and out of the hospital. They see patients independently, collaborate with physicians, and build meaningful relationships to foster trust and understanding of care plans.

Practicing in outpatient settings, they work within a collaborative care team comprising physicians, nurse practitioners or physician assistants, medical assistants, a dedicated medical scribe, registered nurses, and care managers. They also coordinate with specialists and hospitals for comprehensive care. The focus is on primary care, care coordination, and population health.

Core Responsibilities

• Provide exceptional primary care, conduct office visits for routine and acute issues.
• Administer annual wellness visits and health risk assessments with accurate documentation.
• Coordinate care with other providers, specialists, testing facilities, and agencies.
• Lead population health initiatives in collaboration with the care team.
• Assist with phone triage and outreach efforts.
• Educate patients about their health conditions, care plans, and treatments.
• Participate in promotional activities and conduct home visits as needed.
• Perform other duties as assigned.

This role reports to the Center Medical Director and works closely with operational leadership.
