Base pay is $120,000 per year for full-time, with a productivity bonus program that can add up to $80,000 annually. In-person work, collaborative environment, and opportunities to grow in geriatrics and primary care.

• Assessment of all recent admissions and acute issues of both subacute and long-term care patients
• Prevention of needless hospital transfers through early assessment and interventions

• Ability to work as an interdisciplinary team member
• Good critical thinking/ assessment skills and the ability to function in a fast-paced environment
