PHP Nurse Practitioners have a passion for delivering quality care to seniors and other residents in various healthcare settings, including skilled nursing facilities, assisted living, independent living communities, and via telehealth.

PHP is currently recruiting Nurse Practitioners passionate about providing high-quality care for residents at Keyser Healthcare Center in Keyser, WV.

Our Nurse Practitioners work Monday - Friday, 8 am - 5 pm. They are salaried, with some weeks exceeding 40 hours. No call, weekends, or holiday coverage required. Must be willing to work 5 days a week, Monday - Friday.

Job Summary:

The PHP Nurse Practitioner provides high-quality primary care in a person-centered care model. Responsibilities include diagnosing, treating, and managing acute and chronic illnesses through wellness visits, early detection, and timely treatment. The NP collaborates with primary care providers and interdisciplinary teams, involving residents and families in care planning, reducing hospital admissions and ER visits, and fostering a culture of customer satisfaction.

Job duties & responsibilities:
  • Assess, diagnose, and manage chronic and acute conditions.
  • Provide health education.
  • Develop and communicate treatment plans to residents and providers.
  • Order and interpret lab and diagnostic tests; adjust treatment plans accordingly.
  • Maintain compliant documentation in EMR software.
  • Collaborate with operations and clinical leadership to meet goals.
  • Support value-based care initiatives.
  • Ensure excellent customer experience and satisfaction.
  • Practice ethically, following Scope and Standards of Practice and Board Certification.
  • Follow all regulations, guidelines, and laws.
  • Coordinate care with PCPs, interdisciplinary teams, and telemedicine providers.
  • Complete regulatory visits and assessments as required.
  • Participate in family conferences and provide advanced care planning and education.
  • Address pharmacy and medication-related communications.
Qualifications & Experience:

Required:

  • Master of Science in Nursing, DNP, or NP Certificate.
  • National certification (AANP or ANCC).
  • Active, unrestricted state license.
  • Active DEA license.
  • US work authorization.
  • 2+ years clinical experience in geriatric, adult, or family practice.
  • Flexibility in work environment and schedule.
  • Passion for healthcare for seniors and low-income populations.

Preferred:

  • Geriatrics training or experience.
  • 2+ years as a Nurse Practitioner.
  • Experience with ICD-10, HCC coding.
  • Experience in results-driven healthcare models.
  • Willingness to pursue professional development and certifications.
  • Experience in value-based care, outpatient primary care, or skilled nursing settings.

  • Customer service mindset.
  • Excellent time management and prioritization skills.
