Now Offering a $10k Sign-On Bonus for a split position working at Homestead Healthcare Center, Southpointe Healthcare Center, and Wildwood Healthcare Center. (This will NOT mean working at all 3 buildings in one day.)

PHP Nurse Practitioners have a passion for delivering quality care to seniors and other residents in a variety of healthcare settings, including skilled nursing facilities, assisted living, independent living communities, and telehealth.

PHP is recruiting a Nurse Practitioner dedicated to providing high-quality, person-centered care. Our Nurse Practitioners work first shift, Monday - Friday, with no on-call, weekends, or holiday coverage.

Open full-time positions are available at the following locations in Indianapolis:

• Homestead Healthcare Center
• A split position working 4 days at Greenwood Healthcare Center and 1 day at SouthPointe Healthcare Center

Job Summary:

The PHP Nurse Practitioner is responsible for delivering high-quality primary care within a high-touch, person-centered model. The role involves diagnosing, treating, and managing acute and chronic conditions, collaborating with interdisciplinary teams, and engaging residents and families in care planning. This approach aims to reduce hospital admissions and emergency visits, emphasizing customer satisfaction.

Job Duties & Responsibilities

• Assess, diagnose, and manage conditions.
• Provide health education.
• Develop and communicate treatment plans.
• Order and interpret diagnostic tests; adjust treatments accordingly.
• Maintain compliant documentation in EMR software.
• Collaborate with leadership to meet goals and support value-based care initiatives.
• Ensure excellent customer experience and satisfaction.
• Practice ethically, following scope of practice and legal regulations.
• Coordinate care with other providers, including telemedicine.
• Complete required visits and participate in family conferences and advanced care planning.
• Address pharmacy and medication-related communications.
• Maintain presence in assigned facilities during work hours to ensure adequate face-to-face resident care coverage.

Qualifications & Experience Requirements

Required:

• Master of Science in Nursing, DNP, or NP certification
• Certification from AANP or ANCC
• Active, unrestricted state license
• Active DEA license
• US work authorization
• At least 2 years of clinical experience in geriatrics, adult, or family practice
• Flexibility in work environment and schedule
• Passion for healthcare for seniors and low-income populations

Preferred:

• Geriatrics training or experience
• 3+ years as a Nurse Practitioner
• Experience with coding, ICD-10, HCC
• Experience in results-driven healthcare models
• Willingness to pursue ongoing training and certifications
• Experience in value-based care, outpatient primary care, or assisted living settings
