Overview

Boulder Care is looking for Nurse Practitioners who are aligned with Harm Reduction principles and have experience prescribing buprenorphine-based medications for opioid use disorder in an outpatient setting. Responsibilities

• Deliver medication-assisted recovery for Substance Use Disorder (SUD) and empower patients in their recovery journeys in coordination with the Care Team (primarily Care Advocates and Peer Recovery Specialists).
• Provide compassionate telephonic, text-based, and video-based (telehealth) patient engagement and monitoring.
• Support patients with care coordination and treatment plans, in collaboration with Medical Directors/Clinical Leadership, adjusting plans as needed.
• Assist in providing care during scheduled shifts, including some nonstandard hours (evenings/weekends) based on team needs and candidate availability.
• Adopt Boulder’s model of care, SUDs, comorbidities, recovery strategies, and safer use education where appropriate.
• Participate in onboarding and training covering clinical protocols, technology, and workflows.

About The Role

• This position is 100% remote.
• Work directly with patients to deliver medication-assisted recovery for SUD and collaborate with the Care Team.
• Clinical activities include telephonic, text-based, and video-based care delivery.

• Onboarding and training support is provided to ensure day-one success.

Success in This Role

• Patient Facing (80%)
• Maintain an average patient volume around 17 patients per 8-hour day, variable with team needs.
• Evaluate, diagnose, treat, and manage ongoing care for individuals with complex SUDs and related comorbidities.
• Incorporate harm reduction practices into all patient care.
• Assist patients to articulate recovery goals and support them through therapeutic alliance and shared decision-making.
• Educate patients on Boulder’s care model, SUDs, comorbidities, recovery strategies, and safer use where appropriate.
• Coordinate with leadership to develop and oversee treatment plans; provide care coverage during shifts including nonstandard hours.
• Use technology effectively to facilitate a safe and rewarding patient experience.
• Interpret and share lab/clinical results with patients to support goals and safety.
• Enable care coordination with clinical and social resources inside and outside Boulder.

Administrative (20%)

• Participate in department and company-wide meetings.
• Deliver proactive feedback and collaborate with Technology and Operations teams to improve digital treatment platforms.
• Complete thorough chart notes within 24 hours of patient visits.
• Monitor relevant channels for updates and patient care collaboration.
• Follow Boulder’s policies and procedures.

Qualifications

• Minimum 1+ year of addiction treatment experience.
• 3+ years of clinical experience as an NP or RN preferred.
• Degree from an accredited school of nursing and a graduate-level nurse practitioner program.
• Demonstrated experience managing SUDs in a chronic care model with medication-assisted recovery and social needs management.
• Specific experience transitioning individuals from illicit opioids to buprenorphine.
• Proficiency in EMRs and video conferencing platforms (Google Suite or similar highly preferred).
• Ability to gain approval for multiple state licenses and insurance contracts (no significant legal or credentialing barriers).
• Ability to work independently and as part of a team; strong communication and collaboration skills.
• Ability to quickly learn and adapt to new technologies and workflows.
• Strong time management and organizational skills.
• Dedicated, private workspace with a lockable door and high-speed internet to ensure HIPAA compliance.
• Cannot have previously been a patient at Boulder Care.

Schedule & Compensation

• Full-time (30–40 hours/week), Monday–Friday (no weekends).
• Flexible structures: 3x10s, 4x8s, 4x10s, or 5x8s; each shift includes dedicated paid admin time.
• State-specific scheduling options available (examples provided):
• Florida, Kansas, Maryland, North Carolina, Ohio: Typical 8-hour shifts 10am–6pm ET; typical 10-hour shifts 8am–6pm ET; options for shifts ending after 6pm ET; possible mix of Eastern and Pacific shifts.
• Arizona, Colorado, Idaho, New Mexico, Oregon, Washington, Wyoming: Typical 8-hour shifts 10am–6pm PT; typical 10-hour shifts 8am–6pm PT; early Eastern Time shifts not available.
• Compensation: Full-time (1.0 FTE) annualized pay range $130,000–$140,000, prorated for less than 1.0 FTE.
