Job summary

Houston Kidney Specialists Center (HKSC) in Houston, TX, is seeking an Acute Care Nurse Practitioner to join a physician-led nephrology team. The role includes office and inpatient care, patient education, and care coordination within a comprehensive nephrology model.

AGACNP or FNP with Adult Gerontology certs required; TX NP license or eligibility. Bilingual Spanish-speaking candidates are strongly encouraged to apply.

Qualifications

• Master's or Doctoral degree in Nursing from an accredited program.
• Active AGACNP or FNP with Adult Gerontology certification.
• Texas NP license or eligibility to obtain.
• Minimum of one year NP experience.
• Nephrology or dialysis experience preferred.
• Strong communication and collaboration abilities.

Responsibilities

• Evaluate, diagnose, and manage patients with acute and chronic kidney disease.
• Collaborate with nephrologists and care teams.
• Develop and implement treatment plans to improve outcomes.
• Educate patients on kidney health and treatment options.
• Document encounters in the EMR.
• Participate in inpatient and outpatient care coordination.
• Manage a subset of dialysis patients as part of the care model.

Houston Kidney Specialists Center (HKSC) is a growing, physician-led nephrology practice dedicated to delivering exceptional kidney care throughout the greater Houston area. With multiple state-of-the-art clinic locations and a team of experienced nephrologists, HKSC provides comprehensive care for patients with chronic kidney disease, hypertension, dialysis needs, and related conditions.

We are seeking an Acute Care Nurse Practitioner to join our collaborative nephrology team. Whether you are an experienced nephrology provider or looking to expand your expertise into a highly specialized field, we offer comprehensive training and mentorship to help you succeed.

Bilingual Spanish-speaking candidates are strongly encouraged to apply.

You must be an AGACNP or FNP with Adult Gerontology certification for consideration. Experience as an NP is preferred, but not required.

What You'll Do

• Evaluate, diagnose, and manage patients with acute and chronic kidney disease
• Collaborate closely with nephrologists and interdisciplinary care teams
• Develop and implement treatment plans to improve patient outcomes
• Provide patient education regarding kidney health, treatment options, and disease management
• Document patient encounters accurately and efficiently within the EMR
• Participate in care coordination across outpatient and inpatient settings
• Manage a small subset of dialysis patients as part of the practice's comprehensive care model

Schedule

• Monday-Friday
• Primarily office and inpatient-based care
• No routine weekend coverage

Qualifications

• Master's or Doctoral degree in Nursing from an accredited program
• Current Acute Care Nurse Practitioner certification (AGACNP) or Family Nurse Practitioner (FNP) with Adult Gerontology certification
• Active Texas Nurse Practitioner license or eligibility to obtain
• Minimum of one year of Nurse Practitioner experience
• Strong clinical assessment, diagnostic, and care management skillsExcellent communication and collaboration abilities
• Commitment to delivering high-quality, patient-centered care
• Nephrology or dialysis experience preferred, but not required
