Qualifications

• Must be vaccinated and have an active RN and NP license.
• Requires national board certification and current BLS for medical provider.
• 2+ years' experience in care of adult chronically ill patients preferred.

Responsibilities

• Provide healthcare to patients in their home.
• Collaborate with primary care providers.
• Travel within a 50-mile radius from primary residence.

Qualifications

• Must be Vaccinated
• Active, unencumbered RN and NP license in the state of practice
• National board certification in adult, family or geriatric specialties(NP's)
• Has an active DEA license
• Current BLS for medical provider
• Must have valid state driver license and automobile insurance
• Strong computer skills with knowledge working in an Electronic Medical Records system
• 2+ years' experience in care of adult, chronically ill patients preferred
• Must be willing to travel in 50-mile radius from primary residence
