Job Title: Nurse Practitioners (Geriatric Primary Care)

Pay: $125,000-$170,000

Location: Phoenix Locations (2 Openings)

Hiring Status: ASAP

Overview:

• Value Based Care Model (allotting 45 min with each new patient!), serving the senior population. The goal is for providers to build relationships with patients. Work alongside a doctor.
• Prescribe medication and manage non-complex cases.
• Due to the Value Based Care model, the focus is on keeping patients out of the ER. This involves tracking and monitoring patients for check-ups, scans, and preventative care, which may increase administrative work compared to fee-for-service models.
• Depending on the market, walk-in patients may be seen.
• Telehealth options may be available.
• Administrative duties.
• EMR: eClinical Works

Experience Needed:

• Typically, 2-3 years of experience as an NP.
• Experience working with the elderly patient population.
• Experience across multiple specialties such as Internal Medicine, Family Practice, Geriatrics.
• Managed Care experience is a plus.

Nice to Have Skills:

Experience working with geriatric populations, NP experience in Integrative Medicine, Family Practice, Geriatrics, or Pain Management, Managed Care, Bilingual skills, Value Based Care experience.

Soft Skills/Attributes:

• Passion for work and understanding of the served population.
• Flexibility and desire to provide care to the senior population.
• Patience, with a balanced approach to care, spending ample time with patients to build rapport and gather comprehensive data.
