Job summary

Jobtailor is seeking an experienced Nurse Practitioner to collaborate with physicians and families to manage complex adult care plans. You will deliver urgent telehealth and make in-person assessments, coordinating with an interdisciplinary team.

Strong EMR experience and chronic care management skills are essential. Responsibilities include patient education, medication prescribing where permitted, and development of treatment plans aligned with patient goals.

Responsibilities

• Collaborate with physicians, patients, and families to develop complex care plans.
• Provide clinical and non-clinical support to patients.
• Deliver urgent healthcare by telephone and tele-video for home and community-based services.
• Develop and implement clinical care plans for adults with chronic/complex conditions.
• Gather medical histories, perform exams, obtain diagnostics as needed.
• Develop and implement treatment plans aligned with patient goals.
• Identify and close gaps in care.
• Address patients’ needs with interdisciplinary team support.
• Educate patients/families on medications, side effects, illness progression, diet, adherence, and crisis prevention.
• Coordinate care with clinical team and providers.
• Maintain patient records in compliance with regulations.

Job description

• Collaborate with company physicians, patients’ other physicians and providers, and family members to develop complex plans of care
• Provide clinical and non-clinical support to patients
• Provide urgent healthcare by telephone and tele-video to patients receiving home and community-based services
• Develop and implement clinical care plans for adults with chronic and complex conditions
• Gather medical histories, conduct physical examinations, and obtain diagnostics as needed
• Develop and implement treatment plans based on patients’ goals and conditions
• Identify and close gaps in care
• Address patients’ and families’ physical and psychosocial needs with interdisciplinary team support
• Educate patients and families about medications, side effects, illness progression, diet and nutrition, adherence, and crisis prevention
• Coordinate care and resources with clinical team members and medical providers
• Maintain patient medical records and documentation in accordance with regulations and company standards
• Participate in required continuing education
• Prescribe medication as permitted by state authority

Requirements

• Master of Science in Nursing (MSN)
• Active national Nurse Practitioner certification
• Valid, current, active, and unrestricted Family or Adult Nurse Practitioner license in Texas, Iowa, or Kansas
• Valid, current, active RN Compact license
• 2+ years of experience managing complex care cases
• Experience working with Electronic Medical Records (EMR)
• Monday-Friday availability, 8:00am-5:00pm CST, with rotating on-call
• Possession of DEA registration or eligibility preferred
• Active Medicaid number preferred
• Experience managing complex care cases for developmental disabilities and chronically ill patients strongly preferred
