Job summary

Jobtailor is seeking a Nurse Practitioner to provide primary care via telephone and televideo for Home and Community-Based Services patients, collaborating with physicians, patients, and families to develop complex plans of care.

The role centers on adult patients with chronic conditions, requires active NP certification and AZ license (or eligible), EMR experience, and DEA registration where allowed. Strong patient education and care coordination are essential.

Qualifications

• MS in Nursing is required.
• Active NP certification required.
• Arizona license or eligibility in AZ (or TX/KS/IA) and RN license.

Responsibilities

• Collaborate with physicians, patients and families to develop complex care plans.
• Provide clinical and non-clinical support to patients.
• Deliver primary care via telephone and televideo to Home/Community-Based Services patients.
• Develop and implement care plans for adults with chronic/complex conditions.
• Gather histories, exams, and diagnostics as needed.
• Develop treatment plans aligned with patient goals.
• Identify and close gaps in care.
• Address physical and psychosocial needs with the interdisciplinary team.
• Educate patients and families about medications, illness, diet, adherence and crisis prevention.
• Coordinate care with clinical team and providers.
• Maintain medical records and documentation.
• Prescribe medication as permitted by state authority.

• Gather patient histories, physical exams, and diagnostics as needed
• Develop and implement treatment plans based on patient goals and current conditions

• Address patients' and families' physical and psychosocial needs with interdisciplinary team support
• Educate patients and families about medications, illness progression, diet, nutrition, adherence, and crisis prevention
• Coordinate care and resources with clinical team members and medical providers
• Maintain patient medical records and medical documentation
• Prescribe medication as permitted by state authority

Requirements

• MS in Nursing
• Active national NP certification
• Valid, current, active, and unrestricted Family or Adult Nurse Practitioner license in Texas, Kansas, Iowa, or Arizona
• Valid, current, active RN Compact license
• 2+ years of experience managing complex care cases
• Experience working with Electronic Medical Records (EMR)
• Possession of DEA registration or eligibility preferred
• Active Medicaid number in Texas, Kansas, Iowa, or Arizona highly preferred
• Experience managing complex care cases for patients with developmental disabilities and chronic illness strongly preferred
• Required in-person training sessions
• Must reside within a reasonable commuting distance of the posting location(s), unless an accommodation is granted
