Job summary

ChenMed is seeking an experienced Nurse Practitioner to work in Philadelphia, responsible for independently assessing patients, managing care, and collaborating with a multidisciplinary team. Candidates should possess an advanced nursing degree and relevant experience in Family Medicine or Internal Medicine.

This role requires excellent critical thinking, communication skills, and attention to detail, as well as the ability to travel locally 50-75% of the time. The salary range is $111,140 to $158,771, commensurate with experience.

Essential Job Duties and Responsibilities

• Independently assess acute and non‑acute clinical problems, document physical assessments and patient histories, analyze patient trends, and develop and implement a patient management plan. This includes nursing plan development and counseling patients, families, and significant others.
• Plan patient care based on in‑depth knowledge of the patient population and protocols, anticipating and addressing physiological and psychological issues while considering cultural background, level of understanding, personality and support systems. Act as a patient advocate.
• Patient management includes the following:
  • Write admission, transfer, and discharge orders.
  • Order and interpret laboratory and diagnostic studies.
  • Order appropriate medications and treatments.
  • Refer patients for consultation in specialties such as dermatology, neurology, ophthalmology, endocrine, surgery, intensive care, infectious disease, hematology, psychiatry, social service, and dietetics.
  • Document progress notes and summaries in depth.
  • Perform invasive procedures independently once competency is documented.
  • Participate in patient‑care rounds and conferences, communicate management strategies, and collaborate with multidisciplinary team members to ensure successful care.
  • Recognize situations requiring immediate physician attention and initiate life‑saving procedures when necessary.
  • Use advanced communication skills to solve complex problems and improve processes and patient service.

Other Responsibilities May Include

• Collaborate with multidisciplinary team members to analyze and evaluate current healthcare delivery systems and identify and implement new practice patterns.
• Participate in outside activities that enhance personal and professional growth.
• Initiate arrangements and write orders for skilled nursing facility discharges and complete appropriate paperwork.
• Work collaboratively with physicians, nurses, PT, social workers, family and key caregivers to transition the patient to a lower level of care as medically appropriate.
• Introduce yourself to patients/families and explain the nurse practitioner role.
• Facilitate patient/family conferences to review treatment goals, optimize resource utilization, provide family education, and identify post‑SNF needs.
• Enhance collaborative relationships to maximize patients’ and families’ ability to make informed decisions about goals of care, palliative care and hospice.
• Facilitate discharge to appropriate level of care and use preferred providers when additional services are required.
• Prescribe medication to patients in accordance with State practice guidelines.
• Other duties as assigned and modified at manager’s discretion.

Education and Experience Criteria

• ARNP or equivalent advanced nursing degree required.
• Current Nurse Practitioner certification in State of practice required.
• Board certification by AANP or ANCC preferred; may be required for certain States.
• Current DEA number for Schedule II–V controlled substances may be required per State of practice.
• Minimum two years of clinical NP experience in Family Medicine, Internal Medicine, or Geriatrics; prior independent care decision experience required.
• Basic Life Support (BLS) certification from AHA or Red Cross required within first 90 days of employment.

Pay Range

$111,140 – $158,771 salary. Final compensation will depend on experience, education, geographic location, and other factors. The position may also be eligible for bonuses or commissions.
