Job description

Job Overview

The Nurse Practitioner (NP) is part of the clinical operations team and is responsible for providing direct patient care in ChenMed/Jencare medical centers, nursing homes, skilled nursing facilities (SNF), and home settings. The NP may conduct assessments, diagnose conditions, develop care plans, provide health education, refer to physicians and other services, and maintain clear documentation in accordance with ChenMed standards.

Responsibilities
  • Assess and document acute and non‑acute clinical problems, including geriatric assessment, medical history, physical exam, and diagnosis.
  • Develop and implement individualized patient management plans, nursing care plans, and promote patient/family education.
  • Write admission, transfer, and discharge orders; order laboratory and diagnostic studies; prescribe medications and treatments; refer to specialists as indicated.
  • Provide in‑depth progress notes and summaries; participate in clinical rounds, conferences, and multidisciplinary meetings.
  • Perform invasive procedures upon completion of documented competency and when clinically appropriate.
  • Serve as patient advocate, recognizing situations requiring immediate physician attention and initiating life‑saving procedures.
  • Communicate care strategies to the patient care team and collaborate with physicians, nurses, PT, social workers, and family to ensure successful outcomes.
  • Assist in transitioning patients to lower levels of care, facilitate discharge planning, and arrange SNF discharges and related paperwork.
  • Enhance practice patterns through collaboration with multidisciplinary team members and participate in quality improvement and risk management programs.
  • Support professional growth by attending outside activities and continuing education.
Qualifications
  • Advanced Registered Nurse Practitioner (ARNP) or equivalent degree in Nursing.
  • Current Nurse Practitioner certification in the state of practice.
  • Board certification by AANP or ANCC preferred, or required for certain states.
  • DEA number for schedule II–V controlled substances may be required based on state.
  • Minimum of 2 years of clinical experience as an NP, preferably in Family Medicine, Internal Medicine, or Geriatrics.
  • Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required within 90 days of employment.
  • Strong critical thinking, problem‑solving, communication, interpersonal, time management, and organizational skills.
  • Excellent listening and empathetic caring abilities; detail oriented with strong chart‑review skills.
  • Basic computer skills and proficiency with Microsoft Office.
  • Capacity for 50–75% local travel and willingness to work evenings, weekends, and holidays as needed.
Pay Range

$111,140 – $158,771 (base hourly or annual salary; final compensation may vary based on experience, education, location, and other factors). The position may be eligible for bonuses or commissions.
