Job summary

Blue Ridge Care is seeking a dedicated Nurse Practitioner to join the PACE program. You will provide comprehensive, patient-centered care to older adults and collaborate with an interdisciplinary team to coordinate services that promote independence and quality of life.

The role emphasizes holistic, relationship-driven care with opportunities to improve outcomes and participate in program-wide quality initiatives.

Qualifications

• Graduate of an accredited nursing program.
• Current RN licensure and NP licensure.
• National NP certification.
• Minimum two years of experience with elderly or medically complex populations.
• Current CPR certification.
• Valid driver's license and reliable transportation with proof of insurance.

Responsibilities

• Conduct comprehensive assessments and wellness evaluations.
• Provide primary care management for participants with chronic and acute health conditions.
• Develop and update individualized care plans with the Interdisciplinary Team (IDT).
• Collaborate with specialists, hospitals, caregivers, and community resources to ensure coordinated care.
• Educate participants and families on health conditions, treatments, and preventive care.
• Maintain accurate medical records and documentation; participate in quality improvement initiatives.
• Serve as a trusted clinical resource for staff and caregivers.

What You’ll Do

As a key clinical member of the PACE team, you will:

• Conduct comprehensive assessments and wellness evaluations
• Provide primary care management for participants with chronic and acute health conditions
• Develop and update individualized care plans in collaboration with the Interdisciplinary Team (IDT)
• Collaborate with specialists, hospitals, caregivers, and community resources to ensure coordinated care

• Participate in team meetings and contribute clinical expertise to participant care decisions
• Maintain accurate medical records and documentation
• Support quality improvement initiatives that enhance participant outcomes
• Serve as a trusted clinical resource for staff and caregivers

What We’re Looking For

Required Qualifications

• Graduate of an accredited nursing program
• Current Registered Nurse (RN) licensure
• Current Nurse Practitioner licensure
• National Nurse Practitioner certification
• Minimum two years of experience working with elderly or medically complex populations
• Current CPR certification
• Valid driver's license, reliable transportation, and proof of insurance

Preferred Qualifications

• Geriatric Nurse Practitioner (GNP) certification with prescriptive authority
• Two or more years of Nurse Practitioner experience in geriatrics
• Experience in PACE, long-term care, home health, hospice, community-based care, or other interdisciplinary healthcare environments
