The Acute Access Nurse Practitioner, Virtual Care (VSP/PT) works on problems of diverse scope and complexity ranging from moderate to substantial. Nurse Practitioner, Virtual Care (VSP/PT) provides health promotion and maintenance through the diagnosis and treatment of acute illness and chronic conditions. Responsibilities

The Acute Access Nurse Practitioner, Virtual Care (VSP/PT) works in our team-based care environment. The Acute Access Nurse Practitioner, Virtual Care (VSP/PT) will provide care for patients through virtual visits. Visit types conducted will focus on Medicare Wellness visits and acute same day visits.

Patient Care

• The Acute Access Nurse Practitioner, Virtual Care (VSP/PT) will dedicate their clinical efforts to improving access for the patients we serve.
• Evaluates and treats center patients in accordance with standards of care.
• Follows level of medical care and quality for patients and monitors care using available data and chart reviews.
• Acts as an active participant and lead with the care team through daily huddles.
• Helps Associate Medical Director (AMD), Physician and Center Administrator in setting a tone of cooperation in practice by displaying a professional and approachable demeanor.
• Completes all medical record documentation in a timely manner working with a quality- based coder to optimize coding specificity.
• Follows policy and protocol defined by Clinical Leadership.
• Meets with AMD about quality of care, review of outcome data, policy, procedure, and records issues.
• Assures personal compliance with licensing, certification, and accrediting bodies.

Required Qualifications

• This role is considered patient facing and is part of Humana/Senior Bridge's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
