Job summary

Juniper Health, Inc. seeks a Psychiatric-Mental Health Nurse Practitioner to provide psychiatric evaluation and medication management for behavioral health needs in a clinical setting.

You will collaborate with a physician, maintain patient records, and support integrated care under the PCMH model. The role requires a Kentucky PMHNP, national certification, ACLS, and a current Kentucky license, with participation in ongoing education and a driver’s license.

Qualifications

• Master’s degree in Nursing with PMHNP specialization.
• Successful completion of national certification examination.
• Active Kentucky license (unrestricted).
• Must maintain supervisory/collaborative agreements per Kentucky law.
• ACLS certification and ongoing continuing education credits.

Responsibilities

• Perform psychiatric assessments and prescribe psychotropic medications within scope and guidelines.
• Maintain thorough records to support continuity of care and accurate coding.
• Communicate with patients and families about condition and treatment.
• Maintain regular clinic hours and provide patient care during those times.
• Support team-based care and promote collaborative health delivery.
• Advise Clinical Manager/Director of Behavioral Health on behavioral health matters when requested.
• Provide clinical consultation to JHI providers on medication and treatment planning.
• Maintain licensure in Kentucky and stay current with policies and procedures.
• Collaborate with leadership to align health care objectives with JHI mission.
• Attend conferences and trainings to stay updated on integrated care trends.
• Refer patients to appropriate sources when beyond scope and ensure ongoing patient relationships.
• Perform other duties as assigned by supervisor.

Job description

The Psychiatric-Mental Health Nurse Practitioner will provide psychiatric evaluation and medication management for behavioral health issues. The practitioner will also provide case consultation with other providers within the scope of practice of a psychiatric-mental health nurse practitioner to patients in a clinical setting in collaboration with one’s collaborative physician. All Juniper Health employees will adhere to the Patient Center Medical Home (PCMH) model and work to ensure that patients of all ages receive comprehensive, coordinated and patient-centered “whole patient” care. The employee will maintain confidentiality at all times.

ESSENTIAL FUNCTIONS:

• Perform psychiatric assessments and prescribe psychotropic medications within provider’s scope of practice in accordance to prescribed guidelines and clinical protocols.
• Keep accurate records and adequately detailed information to allow: continuity of patient care among providers; quality assurance activities requiring record review; and accurate and truthful coding of diagnosis, services provided, and billing of service charges.
• Communicate appropriately with patients and patients’ families regarding patients’ condition and treatment.
• Keep regularly scheduled clinic hours and provide patient care at all times during these hours.
• Cooperate in promoting the team concept of health care delivery.
• When requested, advise the Clinical Manager, Director of Behavioral Health, Chief Medical Officer, and/or Chief Executive Officer, et.al. on questions involving behavioral health matters.
• Provide clinical consultation for behavioral health issues to JHI providers regarding medication and treatment planning within the scope of the psychiatric nurse practitioner.
• Maintain current licensure in the state of Kentucky.
• Assist in health care planning for JHI. Maintain familiarity with JHI’s mission, goals, objectives, policies, protocols, and/or procedures that are relevant to health care and collaborate with the Director of Behavioral Health, Chief Medical Officer, Chief Executive Officer, and professional staff to assure health care objectives are met.
• Attend conferences, workshops, seminars, and classes to maintain knowledge of changes and developments in the health care industry as they relate to integrated care.
• Refer patients with problems beyond one's professional competencies to appropriate sources of care and maintains a relationship with the patient.
• Perform other duties as assigned by supervisor.

Qualifications

REQUIREMENTS:

• Education level: NURSE PRACTITIONER --Master’s Degree in Nursing, specializing as a Psychiatric Mental Health Nurse Practitioner from an accredited institution approved by the Kentucky Board of Nursing;
• Successful completion of a national certification examination
• Possession of a clean, current, active and unrestrictedlicense in the state of Kentucky
• Must maintain pertinent and appropriate supervisory/collaborative agreements for practice in accordance to Kentucky law
• Must maintain current ACLS certification as well as continuing education credits as deemed necessary for licensure by the Kentucky Board of Nursing/Medicine and one’s certifying bodies.
