Job summary

Adecco Healthcare & Life Sciences is seeking a Nurse Practitioner to join a community-focused primary care organization in San Jose, CA. This is a full-time, direct hire opportunity. The schedule is Monday through Friday, 8:00 AM to 5:00 PM, with occasional on-call rotation.

You will conduct assessments, manage preventive care and chronic conditions, and collaborate with physicians and care teams. A California NP license, DEA registration, and 2+ years of Family or Internal Medicine experience

Qualifications

• Master's degree or higher in nursing and eligibility for NP licensure in California.
• Active California NP license or ability to obtain licensure.
• FNP certification preferred.
• Current DEA registration.
• Minimum 2 years of clinical experience in Family/Internal Medicine or related primary care.
• Strong knowledge of preventive care and chronic disease management.
• Excellent communication and patient education skills.
• Ability to thrive in a collaborative, multidisciplinary setting.
• Bilingual skills preferred (Spanish, Mandarin, Cantonese, Tagalog, Farsi, Vietnamese).

Responsibilities

• Provide comprehensive assessments, diagnoses, and treatment plans for patients across the lifespan.
• Conduct physical exams, review medical histories, evaluate symptoms, and manage acute and chronic conditions.
• Prescribe and manage medications within scope of practice.
• Deliver preventive care and patient education focused on long-term health outcomes.
• Collaborate with physicians and multidisciplinary care teams to coordinate care.
• Refer patients to specialists and ensure appropriate follow-up care.
• Maintain accurate and timely documentation in the EMR.
• Participate in quality improvement initiatives and organizational meetings.
• Support community health programs and patient outreach efforts.
• Participate in an after-hours on-call rotation and provide provider coverage as needed.

Key Responsibilities

• Support community health programs and patient outreach efforts.
• Participate in an after-hours on-call rotation and provide provider coverage as needed.

Qualifications

• Master's Degree or higher in Nursing.
• Active California Nurse Practitioner license, or ability to obtain licensure.
• Family Nurse Practitioner (FNP) certification preferred.
• Current DEA registration required.
• Minimum of 2 years of clinical experience in Family Medicine, Internal Medicine, Pediatrics, or a related primary care setting.
• Strong knowledge of preventive care, chronic disease management, and community health principles.
• Excellent communication, patient education, and critical-thinking skills.
• Ability to thrive in a collaborative, multidisciplinary care environment.
• Bilingual skills in Spanish, Mandarin, Cantonese, Tagalog, Farsi, or Vietnamese are highly preferred.
