Job summary

Soap Creek Resources is seeking a full-time Nurse Practitioner or Physician Assistant to deliver comprehensive primary care in a community health setting in the Greater Sacramento area. The role includes collaboration with physicians and a multidisciplinary team to serve underserved populations.

Compensation ranges from $131,000 to $170,000 annually based on experience, with relocation assistance available.

Qualifications

• Active California RN and Nurse Practitioner license.
• Current NP board certification.
• Current DEA certification.
• Current BLS certification.
• 1–2 years clinical experience, especially in community health or underserved populations.
• Ability to treat three patients per hour.
• EPIC experience.
• In-basket management competency.
• Bilingual in English and Russian/ Farsi/ Pashto/ Spanish preferred.

Responsibilities

• Provide comprehensive primary care in a community health center setting.
• Order and interpret labs, imaging, and diagnostics.
• Diagnose and manage acute and chronic conditions.
• Prescribe medications and plan treatments.
• Develop and implement treatment plans with Medical Director.
• Monitor progress and adjust care plans.
• Initiate emergency procedures when indicated.
• Provide preventive care and health education.
• Maintain accurate documentation per standards.
• Collaborate with physicians and care teams.

Nurse Practitioner

Compensation: $131,000 - $170,000 annually (based on experience)

Job Type: Full-time, Mondays to Fridays 8-5 PM or 4 day (10 hour shift available).

Location: Greater Sacramento area, CA (Multiple locations available)

We are seeking a full-time Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to provide comprehensive primary care services within a community health center setting. This role offers the opportunity to serve diverse and underserved populations while working collaboratively with physicians and multidisciplinary care teams.

Key Responsibilities

• Perform comprehensive patient evaluations, including history and physical examinations
• Diagnose and manage acute and chronic medical conditions
• Order and interpret laboratory tests, imaging, and other diagnostics
• Prescribe medications and therapeutic interventions within scope of practice
• Develop and implement treatment plans in collaboration with the Medical Director
• Monitor patient progress and modify care plans as appropriate
• Initiate emergency procedures when clinically indicated
• Provide preventive care, health education, and counseling services
• Maintain accurate and timely documentation in accordance with agency and regulatory standards
• Collaborate with physicians and other healthcare professionals to ensure coordinated, high-quality care

Qualifications

• Active California RN and Nurse Practitioner license
• Current Nurse Practitioner board certification
• Current DEA certification
• Current BLS certification
• 1-2 years of clinical experience preferred, particularly in community health or underserved populations
• Strong commitment to serving low-income, homeless, or medically underserved communities
• Comfortable treating three patients per hour
• EPIC experience
• In-basket management competency
• Bilingual skills in English and Russian, Farsi, Pashto/Pushto, or Spanish strongly preferred
