What You’ll Do

Perform comprehensive annual wellness assessments, adapting care plans for participants with complex needs, frailty, or higher risks.

Be the go‑to expert for urgent care, same‑day assessments, and seamless transitions of care for participants returning from the hospital.

Work hand‑in‑hand with Primary Care Providers to co‑manage care for high‑risk participants, always seeking ways to fill care gaps.

Lead home care coordination, creating personalized care plans that promote health and independence, with ongoing follow‑up for the best possible outcomes.

What You Bring

A minimum of 3 years of clinical nursing experience (preferably in home care or care management).

Strong background in geriatrics, internal medicine, or primary care with experience working with elderly or frail populations.

A valid NY State Nurse Practitioner license, DEA Certification, and AANP or ANCC Board Certification.

Comfortable providing home care 20%-50% of the time.
