Job summary

Liberty Medical Care Services seeks a Nurse Practitioner or Physician Assistant to provide patient care in local Skilled Nursing & Rehabilitation facilities. You will conduct physicals, manage chronic conditions, and coordinate discharge plans while documenting in the EMR.

The role requires an unrestricted NP/PA license, a DEA certificate, and experience in adult/geriatric or long-term care settings. A $10,000 sign-on bonus is offered.

Qualifications

• Unrestricted NP or PA license in the state of assignment or the ability to obtain.
• Current DEA certificate or ability to obtain.
• Clinical background in adult, family or geriatric specialties preferred.
• Past experience in long-term care or with seniors preferred.

Responsibilities

• Provide nursing services to local Skilled Nursing & Rehabilitation facilities.
• Direct and coordinate nursing care to assure quality patient outcomes.
• Conduct physicals and annual assessments for assigned patients.
• Complete acute visits to assess and diagnose patient conditions.
• Identify changes in condition early and manage chronic diseases.
• Complete H&Ps for new admissions within 72 hours.
• Prepare discharge orders and prescriptions for discharges.
• Encourage advanced care planning and end-of-life decisions.
• Document assessments and care plans in the EMR.
• Collaborate with physicians and staff; notify families of changes.
• Communicate with nursing staff daily; supervise and evaluate performance.
• Review policies for compliance and suggest improvements.
• Identify training needs and coordinate education.

Job Description

• Provides client services to local Skilled Nursing & Rehabilitation facilities.
• Assures quality nursing care for patients by directing and coordinating nursing care.
• Conducts physicals and annual assessments for assigned patients.
• Completes acute visits to assess and diagnose patient conditions.
• Focuses on early identification of changes in condition as well as strong management of chronic conditions.
• Completes history and physical on new admissions within 72 hours of admission or readmission.
• Completes Medicare certifications according to required time frame.
• Prepares discharge orders, instructions and prescriptions for patients being discharged.
• Encourages advanced care planning and end of life decisions for patients requiring these services.
• Provides pharmacy with needed narcotic prescription refills as patient need dictates.
• Documents patient assessment and plan of care in the electronic medical record.
• Reviews labs and other diagnostic tests for additional interventions.
• Collaborates with physicians and staff.
• Notifies family of changes in condition.
• Communicates daily with nursing staff, assigns duties, supervises, and evaluates work performance.
• Reviews nursing policies and procedures for compliance, and to make recommendations for changes/additions as necessary.
• Recommends or conducts specific areas of in-service or continuing education.
• Assesses patients for palliative care needs and creates/oversees the palliative plan of care In facilities and in patient's homes.

Job Requirements

• Current, unrestricted valid Nurse Practitioner license from the Board of Nursing/PA License from the Medical Board in the state of assignment or the ability to obtain.
• DEA certificate current or the ability to obtain.
• Clinical background in adult, family or geriatric specialties preferred.
• Past experience working in long-term care, a nursing home, or with seniors in other settings preferred.
• Strong communication and relationship building skills preferred.
• Intermediate level of proficiency in PC based word processing, including Microsoft Word and Outlook.
• Willing to be 'on call' for Facilities as needed.
• Dependable, flexible, and able to work and cooperate well with all nursing personnel and other departments.
• Ability to make decisions regarding nursing problems, work well under pressure, and problem solve.
