Job summary

LifePath Systems Crisis Center in Plano, TX seeks a Crisis Center Nurse Practitioner to provide psychiatric medication management and coordinate care for individuals with mental health needs. The role operates within state and federal guidelines and participates in interdisciplinary teams.

Shifts are on-site (8am–8pm) with on-call telehealth (8pm–8am). Texas licensure, DEA certificate, and crisis-care experience are required; trauma-informed, person-centered care is emphasized.

Qualifications

• MD from an accredited medical school with TX medical license in good standing or an NP with TX Psychiatric/Mental Health licensure.
• DEA certificate required and unrestricted eligibility for Medicare/Medicaid.
• Experience in a CCBHC or Texas Community Mental Health Center preferred.

Responsibilities

• Provide psychiatric medication management per guidelines.
• Conduct morning and evening rounds at the Extended Observation Unit (EOU).
• Coordinate care with the treatment team and participate in discharge planning.
• Ensure proper laboratory tests and results are ordered and evaluated prior to prescribing/renewing psychotropic meds.

The Crisis Center Nurse Practitioner works within federal, state, and local guidelines to provide psychiatric medication management to individuals who present with a broad range of physical, mental, behavioral, and social needs at LifePath Systems Crisis Center. The complex psychiatric clinical work includes evaluation of individuals for admittance to the Extended Observation Unit (EOU), evaluation and management of medication, and coordination of care with other treatment team members within LifePath Systems continuum of treatment programs. This position will provide morning and evening rounds at the EOU for all current admissions. The Crisis Center Nurse Practitioner will conduct follow-up care, provide input to the person-centered recovery plans, collaborate on discharge planning, as well as participate in compliance with Information Item V requirements. This position will provide assistance with individuals in the Crisis Respite Unit as needed as well as see individuals who were recently in a crisis and needing an evaluation after completion of intake into a full level of care. The Crisis Center Nurse Practitioner will utilize initiative and independent judgment within their scope of practice and may consult with other members of the treatment team or external specialists as necessary. The position will perform complex psychiatric clinical work in a culturally competent, trauma-informed, person-centered method empowering individuals to guide treatment and is responsible for ensuring proper laboratory tests and screenings are ordered and results evaluated prior to prescribing and/or renewing psychotropic medications. Participates in interdisciplinary treatment team meetings as requested and promotes least restrictive alternatives in an attempt to reduce inpatient and law enforcement interventions. The Crisis Center Nurse Practitioner will ensure that all clinical practices are completed in accordance with their scope of practice as recognized by the laws of the state of Texas along with HIPAA and federal guidelines. Other duties as assigned to best fit program needs.

Expectations include timely and thorough documentation, time management and organization skills, active collaboration with Crisis Center workforce members, and strong teamwork abilities.

Service Targets

• Include completing examination within 1 hour of notification from Crisis Center staff.
• Completes reassessment of progress of individuals on the unit each morning and evening.
• All documentation is completed and signed after meeting with the individual.
• 90% of full chart reviews receive a score of 80 or higher.
• Attends monthly team meeting.
• All training is completed by the due date.

Shift Details

Shifts are on site 8am-8pm and on call telehealth 8p-8a.

Due to Crisis Center operating 24/7, employees may be required to work on center holidays if the holiday falls on their scheduled workday. In the event of unforeseen circumstances that require the use of Paid Time Off, employees are expected to notify their supervisor within 4 hours of their regularly scheduled work time.

Qualifications and Experience

• Medical Doctorate Degree or equivalent from an accredited medical school and current licensure from the Texas Board of Medical Examiners that is in good standing OR
• Masters degree from an accredited school of nursing and current licensure for the Texas Board of Nursing as a Psychiatric/Mental health Nurse Practitioner that is in good standing
• Must have a Drug Enforcement Administration (DEA) Certificate
• Unrestricted eligibility to participate in network for Medicare/Medicaid/Other third-party payers
• Experience in another CCBHC or Texas Community Mental Health Center highly preferred
• Experience in psychiatric inpatient facility or other psychiatric crisis management services (Extended Observation, Crisis Residential/Respite, Crisis Stabilization Unit) highly preferred
• Must have reliable transportation and a valid driver’s license

• Knowledge of and strong sills, including Microsoft Office Suite or similar software; and
• Ability to accurately and concisely document information using an electronic health record (EHR).
