Job summary

Mt. Washington Pediatric Hospital in Peoria, AZ seeks a Hospice Nurse Practitioner to provide medical care and manage terminal illness under physician supervision. The role emphasizes low caseload and patient-focused care within OnCare Health Team.

Responsibilities include hospice eligibility determinations, interdisciplinary care planning, compliance with Medicare/Medicaid rules, and completing face-to-face visits as required by hospice benefits.

Qualifications

  • Graduated from an accredited School of Nursing and accredited Nurse Practitioner Program.
  • A master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience.
  • A current DEA license.
  • An active driver’s license, acceptable driving record, and access to reliable transportation.

  • Provide medical care, palliate, and manage terminal illness in compliance with the state Nurse Practice Act under the supervision of collaborating physician.
  • Complete Hospice fact-to-face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based on the hospice conditions of participation and local coverage determinations.
  • Make recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care.
  • Communicate and collaborate with the interdisciplinary group to create, review, and revise the patient's plan of care.
  • Maintain knowledge of and compliance with current Medicare/Medicaid, state/federal rules, and regulations for hospice services.
  • Fulfill the obligation of requested and/or accepted assignments.
