Qualifications

Responsibilities

  • Perform comprehensive wound assessments and develop individualized treatment plans.
  • Provide bedside wound care in post-acute and senior-care facilities.
  • Carry out wound-care procedures within scope of practice.

  • Document thoroughly in the EMR and participate in quality initiatives.
  • Collaborate with facility staff, physicians, and families on treatment.

Position Overview

A growing healthcare organization specializing in mobile and facility-based wound care is seeking an experienced Nurse Practitioner or Physician Assistant to join its Pennsylvania clinical team.

This is a full-time opportunity providing specialized wound care to patients primarily within skilled nursing, long-term care, rehabilitation, and assisted living settings.

Providers work independently while collaborating closely with facility clinical teams, patients, families, and other healthcare professionals. The position is well suited for an APP who enjoys hands-on patient care, autonomy, and developing long-term relationships with post-acute care facilities.

Responsibilities

As a Wound Care NP/PA, you will:

  • Perform comprehensive wound assessments and evaluate contributing medical conditions and risk factors.
  • Develop individualized treatment plans based on each patient's clinical needs.
  • Provide bedside wound care within assigned post-acute and senior-care facilities.
  • Perform appropriate wound-care procedures within your scope of practice.
  • Monitor healing progression and modify treatment plans when clinically appropriate.
  • Utilize evidence-based wound-care protocols and available clinical technology to support patient outcomes.
  • Collaborate with nursing staff, facility leadership, physicians, patients, and families regarding treatment recommendations.
  • Educate facility staff and patients regarding wound prevention, treatment, and ongoing care.
  • Maintain complete and timely clinical documentation within the organization's electronic medical record system.
  • Participate in quality initiatives focused on improving wound outcomes and reducing complications.
  • Travel between assigned partner facilities according to an established territory and schedule.

  • Previous wound care experience strongly preferred.
  • Experience working in skilled nursing, long-term care, rehabilitation, assisted living, or post-acute care environments preferred.
  • Comfortable traveling regularly to assigned healthcare facilities within a defined territory.
  • Comfortable utilizing electronic medical records, laptops, and mobile technology for documentation.
  • Strong communication and interpersonal skills.
  • Ability to work collaboratively while managing a high degree of clinical autonomy.
Territory & Travel

This is a field-based position serving healthcare facilities throughout the Franklin, York, and Cumberland County region of Pennsylvania.

Providers should be comfortable traveling to assigned facilities within approximately a 55-mile radius of their home or designated territory. Mileage and eligible toll expenses are reimbursed.

Ideal Candidate
