Qualifications

Role Description

This full-time remote Nurse Practitioner role focuses on providing specialized wound care to patients in home and long-term care environments. Day-to-day responsibilities include performing comprehensive wound assessments, developing and implementing treatment plans, and monitoring healing progress through telehealth and coordinated onsite visits. The Nurse Practitioner will collaborate with interdisciplinary teams, communicate with patients, families, and facility staff, and ensure documentation is accurate and compliant with regulatory standards. Additional duties include educating patients and caregivers on wound prevention and self-care, participating in quality improvement initiatives, and using digital tools to support remote clinical decision-making.

Qualifications

• Clinical expertise in wound care, including assessment, debridement awareness, dressing selection, and infection recognition.
• Strong skills in patient evaluation, care planning, and ongoing management of chronic and complex conditions.
• Proficiency with electronic health records, telehealth platforms, and remote care coordination tools.
• Effective communication, education, and collaboration skills for working with patients, families, and interdisciplinary teams.
• Current Nurse Practitioner license in applicable state(s), with national certification (e.g., FNP, AGNP) preferred.
