Job summary

Heart to Heart Hospice in Nashville seeks a Nurse Practitioner to provide expert palliative care, educate patients and families, and collaborate with the care team to develop comprehensive plans. You will assess disease progression, manage symptoms, and ensure high-quality end-of-life care with timely documentation.

The role requires an APN and RN license, a BSN, and at least two years of APN experience in hospice or related settings. A valid driver’s license and reliable transport are essential.

Qualifications

• Graduate with BSN from an accredited program.
• Current RN license and APN license in the state.
• Valid driver's license, reliable transportation, and auto liability insurance.
• Two years of APN experience in a clinical setting; hospice or home health preferred.
• Strong knowledge of pharmacology, palliative care, and end-of-life care.

Responsibilities

• Consult with and educate patient/family regarding disease process, self-care techniques, end-of-life care.
• Assist in the IDG’s development of patient/family plan of care to ensure quality care.
• Interact with attending and consulting physicians regarding palliative care in collaboration with hospice physician.
• Inform physicians and staff of changes in patient needs and outcomes of intervention while evaluating response to care.
• Timely submission of all documentation.
• Performance of other duties as required; ensure compliance with laws and policies.

What You Will Do:
