Job summary

Heart to Heart Hospice in Indiana seeks a Nurse Practitioner to join our clinical team. You will consult with patients and families to guide end-of-life care and coordinate plans with the interdisciplinary group to ensure quality care.

The role requires current Indiana RN and APN licenses, a valid driver's license, and at least two years as an APN in hospice or home health preferred.

Qualifications

  • Current Indiana RN and APN licenses required.
  • Graduate of a Baccalaureate School of Nursing.
  • Valid driver's license and reliable transportation.
  • Two years of APN experience in hospice or home health preferred.
  • Knowledge of pharmacology, palliative care and end-of-life.

Responsibilities

  • Consult with and educate patient/family on disease, self-care and end-of-life care.
  • Assist in developing patient/family plan of care with the IDG to ensure quality care.
  • Collaborate with physicians regarding palliative care needs and interventions.
  • Inform team of changes in patient needs and outcomes of care.
  • Timely submission of all documentation.
  • Perform other duties as required and ensure legal compliance.

What You Must Have:
  • Graduate of a Baccalaureate School of Nursing
  • Current State license as a Registered Nurse and as an Advanced Practice Nurse (APN)
  • Current Driver's License, reliable transportation,and provide ongoing valid and current auto liability insurance
  • Two years of experience as an APN in a clinical care setting, hospice or home health preferred
  • Comprehensive knowledge of pharmacology, palliative care, and end-of-life
What You Will Do:
  • Consult with and educate the patient/family regarding disease process, self-care techniques, end-of-life care
  • Consult with and assists in the IDG’s development of patient/family plan of care to assure quality care is provided
  • Interact with attending and consulting physicians as necessary regarding palliative care, in collaboration with hospice physician
  • Inform physicians and other personnel of changes in the patient’s needs and outcomes of intervention, while evaluating patient/family response to care
  • Timely submission of all documentation
  • Performance of other duties as required
  • Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
