True Team Medical in Lumberton, NC is seeking a Nurse Practitioner (FNP or PMHNP) to join our Assertive Community Treatment team. The ideal candidate will provide comprehensive care for patients with severe and persistent mental illness, conducting assessments and coordinating treatment plans.

We are actively seeking a Nurse Practitioner (FNP or PMHNP) or Physician Assistant (PA) to support our Assertive Community Treatment (ACT) Team and Outpatient Program (OPT) in Lumberton, NC. The Assertive Community Treatment (ACT) team consists of a community-based group of medical, behavioral health, and rehabilitation professionals who use a team approach to meet the needs of a patient with severe and persistent mental illness. The ACT team assists a patient in advancing toward personal goals with a focus on enhancing community integration. As an ACT Team psychiatric care provider (NP or PA) you are integrated on the team and provide direct services to patients.
