Comprehensive Rehab Consultants (CRC) is seeking a Nurse Practitioner/Physiatry Clinician to join our post-acute therapy team at a designated SNF in Lebanon, PA. This is a 32-hour, W-2, full-time role with a 4-day work week and hours roughly 9 AM to 4 PM.

You will deliver PM&R services, collaborate with PCPs and therapy disciplines, and drive patient independence while benefiting from competitive salary ($95,000–$125,000), generous bonus, education reimbursement, and comprehensive benefits

Qualifications

• Licensed NP/APRN with PA state license.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC with clinical leadership in facility.
• Collaborate with PCPs to manage SNF conditions (e.g., CVA, neuro conditions).
• Work with PT/OT/SLP teams to improve function and quality of life.
• Follow therapy patients to track progress in therapy.
• Assess and manage barriers to therapy (pain, spasticity, neuro issues).
• Attend multidisciplinary meetings and support culture-building.

This position is for an NP PM&R Clinician with Comprehensive Rehab Consultants (CRC), working with a post-acute therapy team to optimize rehabilitative outcomes and promote patient independence. The role is based at a designated skilled nursing facility (SNF) or post-acute rehab center.

Position Information

Location: Lebanon, PA. This is a 32-hour, W-2 role.

Responsibilities

• Provide specialized PM&R/Physiatry services as a rehab consultant
• Represent CRC in your facility through strong clinical leadership and collaboration with the interdisciplinary care team
• Collaborate with PCP teams to help manage conditions common in SNFs (e.g., CVA, progressive neurological conditions, postoperative orthopedic surgeries)
• Work alongside PT, OT, and SLP teams to improve patient function and quality of life
• Follow therapy patients to track progress in therapy
• Assess and collaborate on the management of barriers to therapy including joint or neuropathic pain, spasticity, neurogenic bowel, and neurogenic bladder
• Become an integral part of the SNF team by attending multidisciplinary meetings and bridging the gap between therapy and internal medicine
• Engage in culture-building opportunities and ongoing education including monthly Didactic lectures, virtual meetings with Regional Team, and annual regional dinners

• 4-day work week with flexible days (between Monday – Friday); specific days can be selected during on-boarding
• Supplemental pay types: Bonus pay
