Job summary

Chestnut Health Company is seeking a Nurse Practitioner to join its physiatry team. This role focuses on Physical Medicine and Rehabilitation to restore function and improve quality of life for patients with injuries or disabilities.

You will collaborate with therapists and nurses to create comprehensive treatment plans, perform thorough assessments, and monitor progress to optimize outcomes and independence. A strong emphasis on education for patients and families is also required.

Qualifications

• Eligible to work in the United States.
• Actively licensed to practice in the state of employment preferred.
• Current DEA License by start date preferred.
• ANCC or AANP certification preferred.

Responsibilities

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
• Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions.
• Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence.
• Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
• Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems.
• Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care.

• Interest in working in Physiatry / Physical Medicine and Rehabilitation is a plus
• Experience in orthopedics is a plus
• Experience in physiatry is a plusExperience in skilled nursing is a plus

License/Certification
