Job summary

Choice Health is seeking a Nurse Practitioner to develop and manage a personal panel of patients receiving community-based palliative care in Amarillo and surrounding areas. You will focus on symptom management and patient education, while participating as a leader and educator within the care team.

The role involves home visits, collaboration with physicians, and ongoing coordination with discharge planners.

Qualifications

• Graduate of a recognized RN program required.
• Master's degree in Nursing required.
• 1-3 years work experience as a Nurse Practitioner required.
• Current license as an APRN by the Texas State Board of Nursing required.
• Valid prescriptive authority from both the TX TMB and Federal DEA
• American Heart Association - Basic Life Support Certificate
• Certification as a Nurse Practitioner by the ANCC or other approved National Credentialing Agency
• Specialty certification in palliative care by ACHPN within 12 months of employment.
• Experience with computer systems including MS Office and web-based applications.

Responsibilities

• Provide community-based palliative care focused on symptom management and work with patients/families to determine their understanding of their condition, its treatment, and their treatment options.
• Confer with the Palliative Care Medical Director or direct supervisor regarding assigned patients.
• Keep physicians and other caregivers informed of needs and problems related to patient care.
• Work with discharge planning entities to enhance coordination of palliative care post-discharge.
• Participate in weekly inter-disciplinary team meetings and others as assigned.
• Cooperate with other personnel to achieve the objectives of palliative care and maintain good employee/interdepartmental/and public relations.
• Represent a cohesive voice of Choice Health in working with medical and hospital staff.
• Participate in and/or provide educational programs on palliative for health professionals and other members of the community.
• Maintain patient rights and confidentiality of patient information, and abides by the Choice Health care standards of excellence.
• Demonstrate knowledge and skills to appropriately communicate and interact with the patients, families, and visitors of all age groups while being sensitive to their cultural and religious beliefs.

This is an awesome opportunity for a Nurse Practitioner who desires the ability to build his or her own panel of patients, and not have to deal with the "business" part of a practice. This position will work as a provider to new and established Choice Health Palliative Care patients in Amarillo and the immediate surrounding area.

Choice Health is a community-based palliative care company. We do not see patients in a clinic but rather we go to our patients, wherever they call home.

The Nurse Practitioner provides community-based palliative care for patients focused on symptom management and works with patients/families to determine their understanding of their condition, its treatment and their treatment options. This position also demonstrates the role functions of consultant, educator, researcher and leader/change agent.

Work Environment

The work of the nurse is mostly performed while the individual is sitting but free to move about at will, in a clinical setting and in the community. To provide requested services in the community, the individual must be able to drive to those areas. The individual must have the visual acuity to allow for close eye work and differentiation of fine details. Must be able to communicate verbally with individuals and groups. Hearing acuity is needed for good listening. Performance of work requires hand coordination and manual dexterity, fine and gross manipulation of object, grasping. A minimal amount of time is spent performing work which requires lifting, carrying, pushing or puling. The majority of work requires mental concentration problem solving, and refining or modifying standard techniques and practices. The environment is mostly clean and comfortable, but may include some annoying factors such as noise, odors, drafts, etc. Possible exposure to blood and body fluids.

• Office, involving frequent interaction with the public, co-workers, and volunteers; regular use of telephones, copy machines, and computers; organization and securing of work documents

Minimum Required Qualifications

Education

Work Experience

• 1-3 years work experience as a Nurse Practitioner required.

Certification/Licensure

• Specialty certification in palliative care by passing the Advanced Certified Hospice and Palliative Nurse (ACHPN®) examination within 12 months of employment.
• Experience with computer systems required, including web based applications and some Microsoft Office applications which may include Outlook, Word, Excel, PowerPoint or Access.

Preferred experience, certification, licensure includes and/or working knowledge in the following:

• Urgent/acute and/or primary care clinical experience (facility and/or community based)

• Adult gerontology, oncology, cardiology, nephrology, pulmonology and related practices/specialties
• Certified Dementia Provider
• Hospice experience
• Long‑term care
