Base pay range

$130,000.00/yr - $150,000.00/yr

Job Duties:

• Work with geriatric patients
• Complete Medicare annual health assessments at clinics and home visits
• Location: Houston
• 7-8 patients/day in clinic
• 3-5 patients/day on home visit days
• This position requires seeing patients in their home, with some appointments in the clinic as needed
• Clinical Responsibilities:
• Perform H&Ps
• Use preventative health screening tools (retinal cameras, A1c testing, kidney testing for diabetics)
• Navigate and chart via EMR systems (eClinicalWorks and others)
• This position focuses on completing comprehensive health assessments and collaborating with patients' primary care providers to ensure continuity of care. You will enhance patient outcomes through detailed evaluations and effective communication, but this is not a primary care or PCP role.

• All equipment provided including laptop, tablet etc

Requirements:

• Current NP license for the state of Texas
• EMR systems and electronic documentation
