Responsibilities

Job Summary:

We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our team at Carease Health. As a Nurse Practitioner with our organization, you will have the opportunity to deliver high-quality preventative care to residents in skilled nursing facilities, while enjoying a hybrid work arrangement that combines on-site visits with remote work from home. Our team is dedicated to improving the lives of nursing home patients through remote monitoring, chronic care management, and psychiatric collaborative care.

Responsibilities:

• Provide preventative care to residents in skilled nursing facilities, including conducting on-site visits and remote monitoring of vital signs

• Collaborate with healthcare teams to develop and implement individualized care plans, while ensuring continuity of care and coordination with other healthcare providers

• Conduct documentation and charting in a timely and accurate manner, with the support of a dedicated scribe team

• Participate in multidisciplinary team meetings and clinical decision-making, utilizing your expertise in Geriatrics and Internal Medicine

• Build strong relationships with facility staff, patients, and families, while maintaining a patient-centered approach

• Stay current with industry developments and best practices through ongoing education and training

Requirements:

• Active NP or PA license in the state of practice

• 1+ year of experience in Geriatric care and documentation review

• Excellent communication, collaboration, and teamwork skills

• Ability to work independently and as part of a team

• Strong problem-solving and critical thinking skills
