Theoria Medical is seeking dedicated physician assistants and nurse practitioners to join its team to provide high-quality care at the skilled nursing facilities (SNFs) it manages. We offer roles suited for providers at various career stages with flexible scheduling in a positive work environment.

Shift Structure

• In-person rounding shifts start around 7AM local time with flexibility within reason.
• Shifts are typically available on Saturday, Sunday, and major holidays.
• This role covers weekend needs and two holidays per year.
• Charting/documentation can be completed off-site.
• This is an in-person position.

Essential Functions and Responsibilities

• Rounding and managing clinical care of the population at the assigned SNF.
• Rounding daily and prescribing medications and treatments for residents at SNFs.
• Providing high-quality, patient-centered, comprehensive primary care services to SNF residents.
• Timely documentation of all patient care/encounters.
• Available for phone/secure messaging consultations with facility nursing staff.
• Documentation completed in Theoria’s proprietary EHR system, ChartEasy.
• Familiarity with PointClickCare, MatrixCare, and American HealthTech EHR systems.

Physical Requirements

• Ability to move inside a facility frequently and briskly to see patients and meet staff.
• Punctual in-person attendance (typically between 7 a.m. and 1 p.m.).
• Operation of app-based EMR/notes system via computer or mobile device.
• Ability to ascend/descend stairs.
• Ability to position oneself to obtain records, tools, or equipment at various heights.
• Effective communication with patients, managers, and staff.
• Ability to observe details within a few feet for accurate medical recommendations and timely updates.
• Must be able to lift/move at least 25 pounds.

Requirements and Qualifications

• Valid PA/NP license in the practicing state.
• Active NP/PA degree from an accredited institution.
• Active DEA license (assistance available for new graduates).
• Post-Acute/SNF experience or knowledge preferred.
• Familiarity with EMR systems and charting practices.
