Qualifications

Active NP license in the state of employment.
Current Controlled Substance License (required on start date).
Current DEA License (required on start date).
ANCC or AANP certification is required.

Responsibilities

Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive rehabilitation plans.
Conduct thorough patient assessments, including histories, exams, and diagnostics, to set rehabilitation goals.
Monitor patient progress and adjust treatment plans to optimize functional outcomes and independence.
Participate in multidisciplinary meetings, conferences and educational activities to enhance clinical knowledge.
Document initial evaluations, treatment plans, progress notes, and discharge summaries in EMR systems.
Educate patients and families about rehabilitation process, expected outcomes, and post-discharge care.

Essential Duties & Responsibilities:

Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs
Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions
Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence
Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills
Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems
Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care
