Salary Range: $110,000.00 Salary/year

ThePrimary Care Nurse Practitionerprovides direct care and education to Health Center patients and ensures continuity and quality of care. Practitioner will have a solid understanding of patient needs and will work collaboratively with a multidisciplinary team to co-manage primary care patients.

Responsibilities

• Provide direct patient care, counseling, and teaching to our primarycare population in anambulatory setting. Care for both acute and chronic conditions for new and established patients.
• Perform complete histories and physical examinations.
• Order, interpret, and evaluate appropriate laboratory and diagnostic tests.
• Develop appropriate plans of care and follow-up based on the outcomes of diagnostic, laboratory, and physical examination findings.
• Order medications and write prescriptions according to organizational and regulatory policies and procedures, including agreement with collaborating physician as required.
• Collaborate and consult with a multidisciplinary team of providers and other staff, including supervising physician, as appropriate.
• Demonstrate expert clinical judgment skills to function in an independent role.

Qualifications

• Graduate of Master's of Science in Nurse Practitioner program or have completed a Certified Nurse Practitioner program as well as Massachusetts licensure and Board Certification.
• Experience with Patient Centered Medical Home model and working with underserved populations
• Knowledge of and experience with EMR and Microsoft Office systems required, withEpic strongly preferred
