We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our team and make a real difference in patient care.

About the Role:

As a Nurse Practitioner with Carease Health, you will be responsible for delivering high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. You will work closely with our experienced clinical team to develop personalized care plans, conduct patients' assessments, and provide medical treatment and education.
