About the Role:

We are seeking a compassionate and experienced Nurse Practitioner to join our team of healthcare professionals. As a hybrid role, you will combine on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. Your responsibilities will include:

• Conducting on-site visits with residents in skilled nursing facilities

• Providing high-quality, patient-centered care and preventive services

• Managing chronic conditions through Remote Patient Monitoring (RPM) and Chronic Care Management (CCM)

• Collaborating with healthcare teams to develop personalized care plans

• Maintaining accurate and timely documentation

Requirements:

• Active NP or PA license in the state of practice

• 1+ year of experience in Geriatric care and documentation review

• Excellent communication, collaboration, and teamwork skills
