Job summary

Carease Health is seeking an experienced Nurse Practitioner (NP) with a geriatrics/internal medicine background to deliver preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team to streamline documentation.

The ideal candidate will have an active NP or PA license in the state of practice, 1+ year in geriatric care, and excellent communication and teamwork skills.
