Job summary

Circadia Health seeks an on-site Nurse Practitioner to drive AI‑assisted bedside care. You will see patients first, act on live signals in real time, and coach facility teams to translate data into early interventions—reducing readmissions.

This full‑time role is in‑field across Dover and Bedford, NH, blending direct patient care with hands‑on training, live demos, and shadowed assessments to optimize care delivery.

Qualifications

• Active NH NP license with ANCC certification (Adult/Family/Geriatric/Acute).
• Eligible for Medicare participation; employer bills on your behalf.
• Minimum of 2 years NP experience with complex adult populations; ability to practice independently.
• Valid NH driver’s license; willingness to travel locally.
• Thrives in a dynamic startup environment.
• English required.

Responsibilities

• Direct patient care: conduct initial and follow‑up assessments; initiate/adjust plans of care; obtain consent; coordinate with facility providers.
• Tech → bedside integration: train and coach staff, integrate Circadia into clinical workflows.
• Interpret trend‑based insights to prioritize high‑risk patients and drive timely interventions.
• Complete documentation and charts within the same day.
• Perform high‑volume rounding across designated SNFs, focusing on new admits and flagged patients.
• Surface field insights to improve workflows and technology integration.
• Mentor and collaborate with NP onboarding and clinical ops/product teams.

About the Role

Transform bedside care with AI. As Circadia’s on-site NP, you’ll see patients first, act on predictive health signals in real time, and coach facility teams to turn data into earlier interventions—reducing preventable readmissions. You’ll blend direct patient care with tech-to-bedside integration through hands‑on training, live demos, and shadowed assessments.

This is a full‑time, in‑field position supporting our partner facilities across Dover and Bedford, NH.

What You’ll Do

• Direct patient care: Conduct initial and follow‑up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers.
• Tech → bedside integration: Train and coach staff at the point of care, standardize use of Circadia into clinical workflows.
• Data into action: Interpret trend‑based insights, prioritize high‑risk patients, and drive timely interventions to prevent decline.
• Close the loop: Complete documentation/charts within the same day
• High‑volume rounding: Efficiently complete visits across designated SNFs (typ. 30+/day where appropriate), focusing on new admits, facility‑flagged patients, and Circadia alerts.
• Quality & product feedback: Surface field insights to improve workflows and tech integration in a clinical setting
• Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams

How Success is Measured (KPIs)

• Visits: Target 30 patient visits/day
• Engagement: ≥80% Time to Acknowledgement Rate: <24h
• Documentation: Complete all notes using AI‑powered tools within 24 hours of each encounter.
• Adoption & impact: Demonstrated workflow adherence at facilities; trend toward fewer falls/readmissions over time.

Must‑Have Qualifications

• Certified and licensed Nurse Practitioner in NH; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid NH driver’s license; comfortable traveling locally.
• Thrives in a dynamic, evolving startup environment.
• English required

• Spanish is preferred.

$130,000 - $160,000 a year

As a full‑time Nurse Practitioner, you will be employed by Circadia Health, Inc. The anticipated annual base salary range for this full‑time position is $130,000 - $160,000. The base range is determined by role and level, and placement within the range will depend on a number of job‑related factors, including but not limited to your skills, qualifications, experience, and location.
