Job summary

Circadia Health is seeking an on-site Nurse Practitioner to join our field team delivering patient-centered care across Dover and Bedford, NH. You will perform initial and follow-up assessments, adjust plans of care, and document findings within AI-assisted workflows, while coaching facility staff to embed Circadia in daily practice.

Qualifications

• NH NP license and ANCC certification required.
• Minimum 2+ years NP experience with complex adult populations; independent practice required.
• Valid NH driver’s license; ability to travel locally.
• English proficiency required; Spanish is a plus.
• Proficient with clinical documentation and data interpretation.

Responsibilities

• Direct patient care: initial and follow-up assessments; initiate/adjust plans of care; coordinate with facility providers.
• Tech bedside integration: train and coach staff on Circadia workflows at point of care.
• Data into action: interpret trend-based insights and prioritize high-risk patients for timely interventions.
• Documentation: complete notes within 24 hours of encounters.
• High-volume rounding: ~30+ visits per day across designated facilities.
• Mentor & collaborate: support NP onboarding and partner with clinical ops/product teams.

Transform bedside care with AI. As Circadia’s on-site NP, you’ll see patients first, act on predictive health signals in real time, and coach facility teams to turn data into earlier interventions—reducing preventable readmissions. You’ll blend direct patient care with tech-to-bedside integration through hands‑on training, live demos, and shadowed assessments.

This is a full-time, in-field position supporting our partner facilities across Dover and Bedford, NH.

• Direct patient care: Conduct initial and follow‑up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers.
• Tech bedside integration: Train and coach staff at the point of care, standardize use of Circadia into clinical workflows.
• Data into action: Interpret trend‑based insights, prioritize high‑risk patients, and drive timely interventions to prevent decline.
• Close the loop: Complete documentation/charts within the same day
• High‑volume rounding: Efficiently complete visits across designated SNFs (typ. 30+/day where appropriate), focusing on new admits, facility‑flagged patients, and Circadia alerts.
• Quality & product feedback: Surface field insights to improve workflows and tech integration in a clinical setting
• Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams

• Visits: Target 30 patient visits/day
• Engagement: ≥80% Time to Acknowledgement Rate: <24h
• Documentation: Complete all notes using AI‑powered tools within 24 hours of each encounter.
• Adoption & impact: Demonstrated workflow adherence at facilities; trend toward fewer falls/readmissions over time.

• Certified and licensed Nurse Practitioner in NH; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid NH driver’s license; comfortable traveling locally.
• Thrives in a dynamic, evolving startup environment.
• English required

• Spanish is preferred.
