Job summary

Columbia Healthcare in Torrington, CT is seeking a full-time Family Nurse Practitioner (APRN) to join its primary care team. The position includes responsibilities across the lifespan in a federally qualified health center dedicated to underserved communities.

The clinic also offers a 24-hour/week opportunity in nearby Winsted that could grow to full-time as patient volume increases. Ideal candidates have active APRN licensure and FNP training with experience in community health or primary care.

Qualifications

  • Active APRN licensure in good standing.
  • Experience in Community Health, Primary Care, Urgent Care, or Emergency Medicine.
  • Family APRN certification preferred.
  • Pediatric experience is highly desirable.
  • Proficient in speaking, reading, and writing English; Spanish is a plus.

Responsibilities

  • Obtain comprehensive patient histories and perform thorough physical examinations.
  • Order and interpret laboratory and diagnostic testing.
  • Diagnose acute and chronic illnesses based on clinical findings and assessment.
  • Develop and implement individualized treatment plans.
  • Provide high-quality, evidence-based primary care to patients across the lifespan.
  • Collaborate with physicians, nurses, MAs, case managers, and patient access representatives to coordinate care.
  • Participate in quality improvement initiatives and interdisciplinary care planning.
  • Practice within areas of expertise and board certification.
  • Support community outreach through health fairs and education programs.

About The Position

A well-established, mission-driven community health organization in northwest Connecticut is seeking a full-time Family Nurse Practitioner (APRN) to join its growing primary care team. This organization provides comprehensive, patient-centered healthcare services to individuals and families of all ages, with a strong commitment to improving access to quality care for underserved populations.

There is also a 24-hour/week opportunity in nearby Winsted, with the potential to grow into a full-time position as patient volume increases.

Ideal candidates will have experience practicing as a Nurse Practitioner in a Community Health Center, Primary Care, Urgent Care, or Emergency Department setting. Family Nurse Practitioner training is strongly preferred, along with pediatric experience; however, candidates with other APRN specialties and relevant clinical experience are encouraged to apply.
