Job summary

Alteas Health is seeking a Cardiology-focused Nurse Practitioner to serve patients across Columbus, OH area in Skilled Nursing, Long-Term Care, and Post-Acute settings. You will collaborate with physicians and interdisciplinary teams to deliver high-quality, patient-centered cardio care.

Responsibilities include evaluating cardiovascular conditions, developing treatment plans, ordering tests, prescribing therapies, and educating patients and families while maintaining compliant documentation.

Qualifications

• Current, unrestricted NP license in applicable state(s).
• Current national certification appropriate to licensure.
• Current BLS certification.
• Minimum six months of NP or PA clinical experience.
• Valid driver's license and ability to travel within territory.
• Experience using Electronic Medical Records (EMR).

Responsibilities

• Evaluate, diagnose, and manage cardiovascular conditions in SNF/Long-Term Care/Post-Acute settings.
• Develop individualized treatment plans using evidence-based practice.
• Order, interpret, and communicate labs, imaging, and other tests.
• Prescribe medications within scope of practice.
• Educate patients and families on diagnoses and care plans.
• Coordinate care with physicians, nursing staff, and other providers.
• Maintain accurate, timely clinical documentation.
• Round at assigned facilities and build relationships with leaders.

Assigned Territory

This position serves patients throughout the Columbus, OH area, providing specialty care at Skilled Nursing, Long-Term Care, and Post-Acute Care facilities located in the Columbus area, and surrounding communities. Daily travel within the assigned territory is expected to support patient care and operational needs. Travel to Dayton, OH one/two times a week.

Providers enjoy a full-time, Monday through Friday daytime schedule with no regularly scheduled weekends or traditional on-call responsibilities, allowing for an exceptional work-life balance.

What You'll Do

As a Nurse Practitioner with Alteas Health, you'll provide specialty Cardiology services while working collaboratively with physicians, facility staff, and interdisciplinary care teams to deliver high-quality, patient-centered care.

Responsibilities include:

• Evaluate, diagnose, and manage acute, chronic, and episodic cardiovascular conditions.
• Develop individualized treatment plans using evidence-based clinical practice.
• Order, interpret, and communicate laboratory studies, imaging, and other diagnostic testing.
• Prescribe medications and treatments within your scope of practice.
• Educate patients, families, and caregivers regarding diagnoses, treatment plans, medications, and ongoing care.
• Coordinate care with physicians, nursing staff, therapy, pharmacy, social services, and other healthcare professionals.
• Maintain accurate, timely, and compliant clinical documentation.
• Round at assigned Skilled Nursing, Long-Term Care, and Post-Acute Care facilities.
• Build strong relationships with facility leadership and clinical teams.
• Represent Alteas Health with professionalism, integrity, compassion, and clinical excellence.

What We're Looking For

Required Qualifications

• Current, unrestricted Nurse Practitioner license in the applicable state(s) of practice.
• Current national certification appropriate to licensure.
• Current Basic Life Support (BLS) certification.
• Minimum six (6) months of Nurse Practitioner or Physician Assistant clinical experience.
• Valid driver's license with reliable transportation.
• Ability to travel daily throughout an assigned territory.
• Strong clinical judgment, communication, organizational, and critical thinking skills.
• Experience utilizing Electronic Medical Records (EMR).

Preferred Qualifications

• Two (2) or more years of Nurse Practitioner experience.
• Experience in Skilled Nursing, Long-Term Care, Post-Acute Care, Internal Medicine, Cardiology, Pulmonology, Hospital Medicine, or Geriatrics.
• Experience caring for medically complex patient populations.
