Responsibilities

• Lead a Patient-Centered Medical Home care team for an assigned panel of patients.
• Provide physical exams or appropriate procedures in a safe manner according to procedure.
• Communicate updated medical information to staff.
• Ensure follow up plans and timely EMR entries.

• Educated with both academic and clinical preparation suitable for Provider role.
