Nurse Practitioner (NP)
At Atlas Medical we are expanding our Home Health & Hospice Provider Team and is seeking a dynamic and experienced individual for the role of Nurse Practitioner or Physician Assistant who shares our desire to make healthcare better for everyone.

Employment Status: Contract w/ Full Time Hours

Compensation: $60-$105/HR DOE

Job Summary
As a Medical Provider for our Atlas Medical Team, you will be entrusted with delivering mobile on-site primary care services to our vulnerable patient population residing in assisted living and other senior communities. This role demands autonomous management of complex healthcare scenarios, including multiple co-morbid conditions. Your expertise will extend beyond medical treatment, as you educate patients and their families on self-care, disease management, and end-of-life care. Collaboration will be a daily aspect of your work, liaising closely with facility staff and Atlas Patient Care support teams. All activities will be guided by quality measures provided by Atlas, emphasizing our commitment to high standards of care. Upholding confidentiality in all job aspects is a non-negotiable requirement, affirming our core values of Character, Experience & Trust.
Desired Skills & Qualifications
Must have at least one (1) year of primary care experience
Valid Licensure as a Nurse Practitioner or Physician Assistant in State of Practice
Ability & Desire to work independently, and drive between communities.
We currently use Elation as our EMR
Effective verbal & written communication & time management skills
Valid AZ Fingerprint Clearance Card
Current CPR Certification & Proof of Negative TB Test

Must have an active Nurse Practitioner license in the state of practice.
Experience
2+ years of clinical experience as a Nurse Practitioner (home health, hospice, primary care, or urgent care experience preferred).
Experience in home-based care, mobile visits, or field-based nursing is highly desirable.
Comfortable working independently in patient homes or residential settings.
Skills & Competencies
Strong clinical assessment and diagnostic skills.
Excellent interpersonal and communication skills—must be able to build trust with patients and families.
Proficiency with electronic health records (EHR) and mobile documentation tools.
Strong organizational and time-management skills—ability to manage a flexible schedule and travel between patient locations.
Knowledge of state and federal regulations, including HIPAA, infection control, and scope-of-practice guidelines.
Other Requirements
Willingness to travel regularly within a defined territory.
Ability to work independently, manage caseloads, and adapt to changing environments.
Physical ability to lift, bend, or perform clinical assessments in non-traditional settings (e.g., patient homes, long-term care facilities).
