Job summary

Som Medical Practice in Niagara Falls, NY is seeking a passionate Nurse Practitioner to join our growing team across multiple locations, including Lockport, Hamburg, Olean, and Niagara Falls. You will deliver high-quality, patient-centered care in a collaborative clinical environment.

Qualifications

• Active NP license in New York.
• MSN or DNP from an accredited program.
• Board certification (FNP-C, ANP-C, or relevant) preferred.
• Valid DEA license and prescriptive authority in New York.
• Experience in a primary care/outpatient setting preferred.
• Proficiency with electronic health record (EHR) systems.
• Strong interpersonal and patient-centered communication skills.

Responsibilities

• Conduct comprehensive patient assessments, including physical exams, medical histories, and diagnostic evaluations.
• Diagnose and treat acute and chronic illnesses in alignment with evidence-based clinical guidelines.
• Develop and manage individualized patient care plans and treatment protocols.
• Order, interpret, and follow up on laboratory tests, imaging, and other diagnostic studies.
• Prescribe medications and provide patient education on medication management.
• Collaborate with physicians, specialists, and care team members to ensure continuity of care.
• Maintain accurate and timely documentation in the electronic health record (EHR) system.
