The Nurse Navigator is the initial point of contact for patients entering the health system. Responsibilities include scheduling, coordinating care, supporting patients throughout the care continuum, and serving as a liaison between patients, families, caregivers, and the multidisciplinary care team. The role demonstrates a commitment to quality patient care and implements innovative practices to meet diverse patient needs.

Key Responsibilities:

  1. Triaging new patient appointments and reviewing medical records for appropriate scheduling.
  2. Assessing barriers to care and referring to support services as needed.
  3. Educating patients on treatment plans and supporting them throughout their care journey.
  4. Collaborating with the healthcare team to outline and implement best treatments.
  5. Maintaining professional knowledge through ongoing education and organization affiliations.

Department Specific Functions include:

  • Assessing patient needs and facilitating timely appointments and referrals.
  • Supporting patient education and navigation through the healthcare system.
  • Participating in multidisciplinary team coordination and process improvements.
