The Nurse Navigator is the initial point of contact for patients entering the health system. Responsibilities include scheduling, coordinating care, supporting patients throughout the care continuum, and serving as a liaison between patients, families, caregivers, and the multidisciplinary care team. Key Responsibilities:

1. Triaging new patient appointments and reviewing medical records for appropriate scheduling.
