Schedule: Full Time – 80 hours/pay period – Monday to Friday – 8 a.m. to 4:30 p.m.

Purpose

Scope
- Deliver white-glove customer service and support within assigned facilities to ensure a high-level of satisfaction and timely transitions of care.
- Collaborate closely with key contacts within strategically assigned facilities to ensure understanding of services and benefits to achieve organizational referral, admission, and census goals and increase utilization of services within assigned accounts.
- Engage in pre-discharge facility calls/visits to prospective patients at the assigned facilities' request upon receipt of the referral to determine appropriateness and deliver seamless care transitions for home health care and hospice business lines.
- Develop and execute annual education plan in assigned facilities and with key stakeholders in partnership with marketing, sales, & clinical teams.
- Manage all aspects of the organization’s care transition program & discharge follow-up calls, as assigned. Partner closely with clinical staff to ensure hospice readiness when appropriate.
Key Responsibilities
- Collaborate closely with key contacts within strategically assigned facilities to ensure understanding of services and benefits to achieve organizational referral, admission, and census goals and increase utilization of services within assigned accounts.
- Create and execute sales plans to achieve goals.
- Document all activities in the organization’s Customer Relationship Management tool.
- Engage in pre-discharge facility calls/visits to prospective patients at the assigned facilities' request upon receipt of the referral to determine appropriateness and deliver seamless care transitions for home health care and hospice business lines.
- Provide “white glove” service in all interactions with patients, families, referral staff, etc., to positively impact the customer experience.
- Ensure consent for services is obtained and complete admission, if appropriate.
- Develop and execute annual education plan in assigned facilities and with key stakeholders in partnership with marketing, sales, & clinical teams.
- Manage the organization’s transition program & discharge follow-up calls, partnering closely with clinical staff to ensure hospice readiness when appropriate.
- Attend weekly ITD meetings to identify patient needs.
- Responsible for identifying and resolving challenges and complaints to ensure client/partner facility satisfaction.
- Occasionally attend events within partner facilities to promote our services.
Education And Experience Required
- 2+ years of healthcare experience as an LPN or RN (RN preferred) within a home healthcare or hospice setting. Must have a current license.
- 2+ years of sales & customer service experience preferred.
- Must have adequate computer skills.
- Must complete and pass a state-required background study.
- Able to physically perform the duties required by the position.
- Must be able to read, write, and speak English well enough to communicate with staff, patients, families, and facilities.
